If you would like to speak with our team, you can call between 9:30 AM – 1:30 PM Monday, Wednesday or Thursday. Alternatively, Contact Us by submitting an enquiry or client referral online. 

What happens next?

After an enquiry or referral is received, we will call you and/or the client to do an intake.

Intake phone call

A 15-minute intake phonecall will take place to help us understand your individual needs and availability.

Appointment booking

An appointment will be booked for the next available at a time that meets your availability.

Important Documents

An email will be sent with a link to a New Client Details form and a Service Agreement for NDIS Participants. We ask that form be completed prior to the appointment.

Norwest Clinic

The Dietitian’s consult at the Norwest Clinic on Wednesday and Saturday.

There are visitor car parks out the front of the building. Alternatively, Coles car park is a short walk away.

Current wait times for an initial appointment are between 6-8 weeks. For urgent cases, we will do our best to find something sooner at the request of a Healthcare Practitioner.

Client Referrals

To refer a client, complete the client referral form. We will review the information provided and contact you and/or the Client during Administration hours.
